I/We shall comply with all of the provisions of the Mendham Township Ordinances. 1. Date tree removal to commence _____________________ Date tee removal to conclude_____________________ TREE PRESERVATION AND LANDSCAPE REGULATIONS 23-15 Penalties Any person who violates any provisions of Sections 23-4 and 23-5 of Chapter XXIII and any person who shall fail to comply with an order issued pursuant to Sections 23-10 or 23-11 of Chapter XXIII or the terms and conditions of any tree removal permit issued pursuant to Chapter XXIII shall be subject to a fine of not more than $1,000 for each and every offense. to the I. The application shall be accompanied by a dimensioned lot diagram with a scale of not Less than one (1) inch = thirty (30) feet. II. The following information is required on the diagram A. the location of all trees to be removed B. dimensions and lot lines C. location of the dwelling and any accessory structures with dimensions D. driveway location E. septic location and size F. elevation around the dwelling and elevations of the lot G. type of soil H. location of well III. Please note: any tree within 15 feet of the dwelling or an accessory structure should be removed. IV. At the work site, the following outer perimeters should be marked with clearly visible Tape: A. foot print of the dwelling B. septic system C. trees to be removed outside these two perimeters should be marked including Those for the driveway and parking areas.
